Conference Evaluation

The Membership Committee is asking your participation in evaluating the success of this Conference. Your comments will help us to monitor the effectiveness of our work and help those planning events in the future to provide you with an improved service.

The conference evaluation is available for download from this web page. Simply print it out, complete it, and mail or fax it back to the ISPA office as instructed on the form (the download is in Adobe Acrobat format, and will require the free Acrobat reader software to view and print).

Please be assured that all of the information you give will be treated as strictly confidential, and your questionnaire will not be passed on to any third party.
