Delegates can register for the NY05 Congress by downloading the registration form below, and faxing it to the ISPA office.

Please note: EARLY BIRD deadline: October, 22, 2004.

CANCELLATION POLICY
Registrations cancelled before October 22 will be refunded, subject to a $50 administration deduction.
Registrations cancelled after October 22 (but before December 23) will be refunded 50%.
Registrations cancelled after December 23 will not qualify for any refund.

FUNDS TRANSFER POLICY
Please note the clause at the bottom of the Registration Form:
Wire Transfer: The transfer must be fully prepaid so that the amount received is the correct amount due in US dollars. If you are not able to arrange for wire and exchange rates to be prepaid, please add 5 percent to the amount of the invoice being paid. To enable us to credit payments correctly, please send a copy of your wire transfer order. The transfer information for our bank account is:
